Used and refurbished ophthalmic lane equipment
Complete lanes come to market when a practice consolidates. The mechanical instruments age gracefully; it is the diagnostic imaging that needs the hard questions.
Ophthalmic lane equipment covers phoropters, slit lamps, examination chairs and instrument stands, autorefractors and auto-keratometers, tonometers, lensmeters, retinal and fundus cameras, optical coherence tomography, visual field analyzers, corneal topographers, A-scan and B-scan ultrasound, and phacoemulsification systems.
The category divides cleanly in two, and the two halves need completely different evaluation. Mechanical and optical instruments — phoropters, slit lamps, lensmeters, chairs and stands — are precision devices with almost nothing to go wrong in them. They age extremely well and a well-kept instrument twenty years old performs like a new one. Diagnostic imaging instruments are computers with optics attached, and everything that matters about them is software, licensing and data.
What varies most between two units that look identical
On diagnostic imaging, the software generation. OCT, visual field analyzers, topographers and fundus cameras run acquisition and analysis software that is versioned, licensed and sometimes tied to the hardware generation. Which version a unit runs decides which analysis modules are available, whether it will still receive updates, and whether it will connect to anything else you own. Confirm the version and confirm which analysis packages are licensed on that serial number.
Whether the workstation is included, and what is on it. Many diagnostic instruments are sold as the head alone, without the computer that drives them, and the computer is not a commodity — it runs a specific operating system version the acquisition software requires. Confirm whether the workstation, its monitor and its software licence are all part of the sale.
Patient data migration. This is the question people discover too late. An instrument arriving full of the previous practice's patient database is a problem, and an instrument whose database cannot be migrated to your systems is a limitation you will live with for years. Ask what happens to the existing data, ask whether the database can be exported in a usable form, and ask whether the manufacturer supports migration at all. Confirm, separately, that the seller removes any existing patient information before the unit ships.
On mechanical instruments, optical condition and movement. Fungus, haze, delamination and scratches inside the optical path are the disqualifying faults, and they are visible if you ask for a photograph taken looking through the instrument rather than at it. Everything else is mechanical: smooth movement through full travel, positive detents, illumination working across its full range, and no drift.
On phaco systems, the consumable chain. Handpieces, tubing packs and cassettes are proprietary and specific to the platform, and the platform is useful only for as long as the packs are still manufactured and available to you. Confirm which handpieces are included and their condition, and confirm the consumables remain in production and can be supplied to your setting.
What to confirm before buying
- Software version and which analysis packages are licensed on that serial number
- Whether the workstation, monitor and software licence are included
- Whether existing patient data can be migrated, and that it is removed before shipment
- Optical condition, from photographs taken through the instrument
- That every mechanical movement runs full travel smoothly and holds position
- Illumination condition and whether bulbs are still available for that model
- On phoropters, that every lens and prism setting operates and the reading rod and accessories are included
- On chairs and stands, powered movement under load and the combination's electrical requirement
- On phaco, which handpieces are included and whether consumable packs remain manufactured
- Whether the manufacturer will support or re-register the equipment under a new owner
- Calibration state, and what recalibration on arrival involves
Freight, delivery and installation
Ophthalmic instruments are precision optics on delicate mounts and they are the category where packing quality matters most relative to size. A slit lamp or a phoropter is small, light and easy to ship — and easy to ruin in transit if it is not immobilised properly inside the box. Ask specifically how each instrument will be packed. Instruments with moving heads should be locked or braced for transport, and anything with a counterbalanced arm should travel with the arm secured.
Chairs and instrument stands are the heavy items in a lane and ship crated on a pallet. The chair-and-stand combination is usually wired as a unit with a single electrical connection, so confirm the room's circuit and the position of the outlet before delivery rather than after.
Diagnostic imaging instruments travel on their own pallets and need level, stable positioning on arrival. Most need recalibration after being moved, which is normal and should be scheduled with the delivery rather than arranged afterwards.
Where a complete lane is being bought from one practice, ship it as one consolidated movement. It costs materially less than moving eight instruments separately, everything arrives together so the room can be commissioned in one visit, and the equipment arrives already known to work together — which is the main advantage of buying a lane intact in the first place.
