What comes to market
Ophthalmic instruments leave US practice through the same events as other hand instruments, but on a faster cycle. Cataract-heavy surgery centers refresh micro sets on a schedule because tips wear. Practices close, merge into ophthalmology groups or drop a subspecialty. A center converting to a different phacoemulsification platform releases trays built around the old technique. Sets also break up when one instrument is damaged and the rest moves on as a lot.
The families that circulate are the ones every anterior-segment tray carries: lid specula in the Barraquer, Lieberman and Kratz-Barraquer patterns; capsulorhexis and Utrata forceps; McPherson and Kelman-McPherson tying forceps; Colibri-type fine-toothed forceps; Castroviejo and Barraquer needle holders, locking and non-locking; corneal and Vannas scissors; muscle hooks, chalazion clamps and lid plates; irrigating and anterior chamber cannulas; lens manipulators, choppers and hooks. Oculoplastic and lacrimal instruments move alongside them — punctum dilators, probes, Desmarres retractors.
Identification starts with a warning. Storz on an ophthalmic instrument is not Karl Storz. The ophthalmic Storz line began with an American instrument company and trades today under Bausch & Lomb; Karl Storz is a German endoscopy manufacturer with no connection to it. Confusing the two is among the most common sourcing errors in this trade, and it runs both ways: buyers searching for endoscopes are shown eye instruments, and genuine ophthalmic lots are described under the endoscopy brand. Read the mark. The ophthalmic instruments carry the Storz name with a letter-prefixed catalog number that looks nothing like Karl Storz's numbering, and the instrument types — specula, capsulorhexis forceps, lens hooks — belong to no endoscopy catalog. If a seller cannot say which company made an instrument, treat the rest of the description as unverified.
Condition language applies as elsewhere — as-removed, reprocessed, refurbished — with one caveat. Refurbishing a micro-instrument means fine tip work under magnification, and few houses do it properly. An instrument called refurbished with no named repairer has unknown tip geometry.
What to check before buying used
- Which Storz. First, confirm from the etch and catalog number that you are looking at the ophthalmic line and not endoscopy hardware. Photographs of the mark, not the seller's title, settle it.
- Magnified tip inspection, always. These instruments cannot be judged by eye or from a photograph taken at arm's length. Use a stereo microscope or high-power loupe, and ask sellers for images shot through magnification. Every check below assumes it.
- Spring set. Castroviejo- and Barraquer-style spring handles must open fully to their rest position and close with even, predictable pressure. A spring that has taken a set closes short, feels dead, or needs a shove to start; that instrument is finished for fine work.
- Tying forceps platforms. Tips must meet flat along the whole platform, without crossing and without a gap that passes light. Draw a suture strand through the closed tips; it should hold and release cleanly.
- Toothed forceps. On Colibri-type fine-toothed forceps the teeth must interdigitate exactly. Bent, blunted or mismatched teeth cannot be corrected in the field and will tear tissue.
- Corneal and Vannas scissors. The blades must shear to the very tip and show no separation when closed. Look for nicks in the edge and a pivot loose enough to let the blades ride apart.
- Needle holders. Confirm the jaws meet along their working surface and that a locking holder engages and releases without snapping the tips together, which is how most locking micro holders are ruined.
- Specula. On wire and blade specula, confirm the blades sit parallel and unbent, the mechanism runs smoothly, and any locking screw holds without stripping.
- Cannulas and lumened instruments. Confirm patency, check for a burr or flare at the tip, and be sure the item is genuinely reusable rather than single-use. Retained residue in ophthalmic lumens is associated with toxic anterior segment syndrome — clean strictly to the current instructions for use and your society's ophthalmic reprocessing guidance.
- Blade and diamond instruments. Confirm any guard retracts and returns. Diamond knives cannot be judged outside a laboratory and should be bought only subject to inspection by the manufacturer or an authorized service house.
- Corrosion and metallurgy. Look at joints, box locks and the handle-to-shank transition, where residue collects. Titanium instruments behave unlike stainless and ignore a magnet. Pitting on any working surface is disqualifying.
- How the lot is packed. The practical filter. Micro-instruments sold loose in a bag or tumbled in a tray should be assumed to have tip damage until magnification proves otherwise. Insist on individual tip protectors, a silicone-fingered tray or a fitted micro case.
Buying considerations for international buyers
Sellers ship from within the United States, and lots are typically offered FOB or FCA at a named US place, with your forwarder arranging onward carriage. Ophthalmic sets are small and light, so most consignments move as air parcels rather than freight, which makes the packing specification more important than the incoterm. Write into the order that each instrument travels in its own tip guard inside a rigid case.
Paperwork normally means a commercial invoice, a packing list, a country-of-origin statement and any instructions for use or repair records the seller holds — repair history matters more here than in any other instrument class, because it tells you who has been near the tips. A manufacturer warranty will not transfer, and a US-market instrument carries no CE mark; US clearance has no standing with your regulator.
Questions for your customs broker and health authority before you commit:
- Whether used or refurbished devices may be imported at all, and under what conditions.
- Whether ophthalmic instruments require registration or notification before arrival.
- Whether a conformity assessment or pre-shipment inspection applies.
- Whether you need an FDA Certificate to Foreign Government or Certificate of Exportability, both requested on the US side ahead of shipment.
- How used medical goods fall for duty and VAT under your tariff schedule.
- Whether a qualified micro-instrument repair service exists near you, since these instruments need one and shipping them back across a border for tip work is rarely practical.
