Used and refurbished anesthesia and respiratory equipment

An anesthesia machine and its vaporizers are frequently sold as separate items. Reading which is which is most of the work of buying one.

Anesthesia and respiratory covers anesthesia machines and workstations, vaporizers, ventilators, breathing circuits and absorbers, gas scavenging equipment, suction, oxygen concentrators, nebulizers and airway equipment.

Anesthesia machines have long service lives and an active secondary market across surgical, dental and veterinary settings. A machine built fifteen years ago and maintained properly delivers gas exactly as accurately as one built last year, which is why the category holds up. The complication is that "an anesthesia machine" is not one item. It is a frame with a gas delivery system, a ventilator, an absorber, a monitor and one or more vaporizers, and any of those can be present, absent, or a different generation from the rest.

What varies most between two units that look identical

Which vaporizers are mounted, and for which agent. This is the first question, and it is frequently the largest single variable in the purchase. Vaporizers are agent-specific — sevoflurane, isoflurane, desflurane — and they are separate items with their own value that are routinely listed apart from the machine they sit on. A photograph showing two vaporizers is not a commitment that two vaporizers are included. Ask explicitly which vaporizers are part of the sale, for which agents, and in what condition.

The vaporizer mounting system. Manufacturers use different mounting standards, and a vaporizer that fits one machine will not necessarily mount on another. Confirm the mount type on the machine and the mount fitting on any vaporizer before assuming the two go together.

The absorber design and its consumables. Absorber assemblies differ by machine generation, they are the part most exposed to moisture and soda lime dust, and the canisters they take are model-specific. Confirm the absorber is present, intact and complete, and confirm which canister style it uses and that the canisters are still readily available.

Gas configuration and the ventilator generation. Which gases the machine is plumbed for, whether cylinder yokes are fitted and for which gases, whether the pipeline hoses are included and terminate in the fittings your facility uses, and which ventilator generation the machine carries — ascending bellows, descending bellows or piston. Ventilator generation determines which ventilation modes are available at all.

Software generation and monitoring integration. On workstation-class machines, the software generation determines available modes and which monitoring integrates with the machine rather than sitting beside it. Confirm the version, and confirm whether the integrated monitor is included and what parameters it carries.

What comes with it. Breathing circuits, rebreathing bags, hoses, cylinder yokes and wrenches, scavenging hoses, and the auxiliary flowmeter where one is fitted. These are ordinary consumables individually and a meaningful cost together.

What to confirm before buying

  • Which vaporizers are included, for which agents, and whether their mounts fit the machine
  • Absorber presence and condition, and which canister style it takes
  • Cylinder yokes fitted, and for which gases
  • Pipeline hoses included, and whether the fittings match your facility's gas outlets
  • Ventilator generation and the ventilation modes actually available on that unit
  • Software version, and which monitoring is integrated versus separate
  • Whether the machine passes its own power-on self-test and leak test
  • Service history and whether the manufacturer will support the machine under a new owner
  • What commissioning, leak testing and documentation your setting requires before first use
  • Electrical requirement and battery backup condition
  • On ventilators, which patient circuits, sensors and consumables are included and still manufactured

Freight, delivery and installation

Anesthesia machines are top-heavy, wheeled and more fragile than they look. They ship crated and palletised, and they should be shipped with the vaporizers removed and packed separately — a vaporizer left mounted through a freight journey is a vaporizer that may need servicing on arrival. Confirm how the seller intends to pack the machine before it moves, and confirm the vaporizers travel in their own protective packaging.

Delivery access is rarely the constraint; the machine rolls once it is uncrated. What matters at the destination is the gas supply. A machine plumbed for pipeline gas needs pipeline outlets with matching fittings, and one intended to run on cylinders needs the right yokes and a plan for cylinder handling and storage. Scavenging is the piece most often overlooked: waste anesthetic gas has to go somewhere, and the scavenging arrangement at the destination has to exist before the machine is used rather than after.

Budget for commissioning. Any anesthesia machine that has been moved should be leak tested, calibrated and documented before it is used on a patient, and vaporizers should be checked for output accuracy. That work is a normal and expected part of buying one used, it is done by a qualified service provider, and it is far cheaper to schedule alongside delivery than to arrange separately afterwards.
