Used and Surplus Flexible Endoscopes
Flexible endoscopes are the highest-stakes purchase on this exchange. A gastroscope, colonoscope, bronchoscope or duodenoscope is a patient-contact device with internal channels, a sealed insertion tube and a reprocessing burden that follows it for life, and a defect that looks like nothing can make it unusable in a compliant department. This page covers what circulates in the United States, the inspection sequence that matters, and the import questions to answer first.
What comes to market
Flexible scopes leave US facilities on a predictable cycle. A GI lab upgrades to a newer processor generation and the previous scopes lose their video partner. A hospital consolidates endoscopy onto one platform. A pulmonary service moves to single-use bronchoscopes and the reusable fleet idles. Endoscopy centers close or change hands. Much of what circulates also passes through repair houses, which take in retired scopes for parts and return what they restore.
Olympus dominates US gastroenterology — gastroscopes, colonoscopes, duodenoscopes and their matching processors and light sources — with Pentax and Fujifilm holding meaningful shares of the same departments. Karl Storz and Richard Wolf flexible cystoscopes, ureteroscopes, bronchoscopes and ENT nasopharyngoscopes circulate steadily, as do flexible ureteroscopes from Stryker and endoscopy accessories from ConMed. Reprocessing hardware from Steris and other vendors moves alongside the scopes, and it matters: a scope you cannot reprocess to its own instructions is not usable.
Three condition descriptions apply and they are not interchangeable. As-removed means decontaminated and pulled from service with no functional assessment — assume nothing about the seal. Function-tested means someone leak tested it, powered it on a compatible processor and confirmed image, illumination, angulation, air, water and suction. Refurbished means a repair house replaced components: bending rubber, insertion tube, channels, imager or fiber bundle, angulation cables. Ask what was replaced and by whom. FDA distinguishes servicing a device from remanufacturing it, and substantial work on a flexible endoscope can cross that line — which determines who carries regulatory responsibility for what you receive.
What to check before buying used
- Leak test first, before anything else. Nothing else here matters if the scope will not hold pressure. Pressurize with the manufacturer's leak tester, confirm the gauge holds, then submerge and angulate the distal end through its full range watching for bubbles. A slow leak that appears only under angulation is the classic failure, and a dry test hides it.
- Channel integrity and patency. Flush and brush every channel with the correct brush diameter, confirm flow from each port, and inspect the returning brush for shredded bristles or debris. Ask whether the channels have been borescoped — a channel image is the most informative document a used-scope seller can supply.
- Bending section rubber. Under angulation, look along the rubber for pinholes, blisters, thin spots, adhesive lifting at either bond line and softness from chemical attack. Most leaks start here.
- Angulation in all four directions. Run the up/down and left/right controls to their stops. Motion should be smooth and symmetric with firm cable tension, no dead band at center and no gritty feel, and the tip should hold position under light resistance. Slack cables and lost deflection are wear, not adjustment.
- Distal tip. Examine the objective lens for scratches, cracks and residue, the light guide windows for darkening, the air/water nozzle for blockage and the channel outlet for burrs.
- Insertion tube. Run the tube through your hands along its length, feeling for kinks, buckling, flat spots, sheath delamination and the crescent marks left by a bite block failure.
- Working channel scoring. Scored or gouged channel walls trap material and cannot be reliably cleaned. Scoring comes from accessories forced through a deflected tip and is grounds to reject the scope.
- Air/water and suction valve seats. Fit the correct valves, check the seats for pitting and scale, and verify insufflation, lens wash and suction respond crisply. Confirm the valve set and water bottle cap are included.
- Connector and video pins. Inspect the light guide connector for bent or recessed pins, corrosion, a damaged venting cap and a deformed light post, and confirm the water-resistant cap that permits immersion is present.
- Duodenoscope elevator mechanism. Work the elevator through full travel, inspect the recess and wire channel, and confirm the model's design — disposable distal cap, sealed elevator or otherwise — matches what FDA and the manufacturer now expect for it.
- Reprocessing instructions you can actually follow. Read the current manufacturer reprocessing instructions against your own department: detergents, brush sizes, soak times, drying, storage, and whether your reprocessor has a validated connection kit for this exact model. No supported connector means an unusable scope whatever its condition.
- Compatibility and service history. Confirm the connector generation mates with the processor and light source you own and the processor recognizes the model, then ask for repair records, the last leak test result and why the scope left service.
Buying considerations for international buyers
Sellers here are US-based, and sales normally move on FOB or FCA terms at a named US location, with your forwarder taking the goods onward. Agree the named place and freight booking early, and ask for the scope to travel in its factory case; one shipped loosely coiled or over-tightened can arrive with a new leak.
Expect a commercial invoice, packing list, country-of-origin statement, and whatever reprocessing instructions and service records the seller has. What generally will not travel: a transferable manufacturer warranty, and CE marking — a device supplied to the US market carries US clearance only, which grants no standing under your national rules.
Confirm the following with your own customs authority and health regulator before you buy:
- Whether used or refurbished endoscopes may be imported at all; several jurisdictions treat patient-contact endoscopes more restrictively than other used devices.
- Whether registration or national listing is required before arrival.
- Whether pre-shipment inspection or a conformity assessment applies.
- Whether an FDA Certificate to Foreign Government or Certificate of Exportability is needed; request it on the US side before shipment moves.
- How duty and VAT are applied to used medical goods in your tariff schedule.
Two subject-specific points. If a video processor or light source travels with the scopes, check its rated mains input and frequency against your supply and ask whether language, region or feature settings are locked in software. Second, establish that authorized or competent independent service exists in your country for that scope family — the repair relationship, not the purchase, keeps a flexible endoscope in service.
